Vale of Glamorgan council issued an unreserved apology to the family for “a serious error of judgment” in placing the teenager with them, despite his background.
The teenager was jailed indefinitely in February at Cardiff Crown Court after he admitted the rape of the two-year-old boy and the abuse of the nine-year-old girl.
In a subsequent interview, the children’s parents said they did not know about the teenager’s past sexual history, and felt “betrayed”.
An inquiry published by the council on Monday found mistakes were made by staff concentrating on the teenager’s own vulnerability and not the risks posed.
“Seen for the most part as a victim in many aspects of his life, he was considered to be the one in need of protection,” the report said.
“Because of this focus, much of the relevant information was not made available to the parents of the children and to some of the staff involved in making decisions about providing accommodation.”
